Q16. Can You Describe the Various Stages of a Pressure Sore?

“From my previous experience as a CNA, I’ve learned that pressure sores, also known as bedsores or pressure ulcers, develop in four main stages, primarily affecting individuals who are bedridden or immobile for extended periods.

Stage 1: The first stage is the mildest. The skin isn’t broken at this point but it may appear reddened on lighter skin, or differently shaded on darker skin, and does not blanch (turn lighter) when pressed. The area might be tender, painful, or warmer or cooler compared to the surrounding skin. Early detection and preventive care are crucial at this stage to prevent progression.

Stage 2: In the second stage, the skin breaks open, wears away, or forms an ulcer, which is usually tender and painful. The sore expands into the deeper layers of the skin and can look like a scrape, blister, or a shallow crater. At this stage, proper cleaning and dressing of the wound are vital to promote healing and prevent infection.

Stage 3: This stage involves the full thickness of the skin and may extend into the subcutaneous tissue layer that lies just below the skin. The sore develops into a deeper crater, with potential for fat visibility in the wound. Management includes more extensive wound care and often requires the expertise of healthcare professionals to manage infection and other complications.

Stage 4: The most severe stage, where the sore is very deep, reaching into muscle and bone, and causing extensive damage. Damage to deeper tissues, tendons, and joints can occur. Treatment for stage 4 pressure sores is complex and can include surgery, advanced wound therapy, and significant interventions to relieve pressure and treat the wound.

In addition to these stages, there are also ‘Unstageable’ pressure sores, where the bottom of the sore is covered in yellow, tan, grey, green, or brown material and cannot be seen. Another category is a ‘Deep Tissue Injury’ where the skin is intact but the tissue underneath is damaged from pressure.

In my role, continuous monitoring for signs of pressure sores, regular repositioning of the patient, maintaining skin hygiene, ensuring proper nutrition, and using supportive devices are integral parts of care to prevent the development of these sores or manage existing ones effectively.”
